Job Description

An excellent opportunity has arisen for a dedicated, dynamic and enthusiastic pharmacist to join the Pharmacy team at Wythenshawe Hospital and Trafford General Hospital. These sites form part of the larger Manchester University Hospital Foundation Trust (MFT) which is one of the biggest, most ambitious and progressive NHS Trusts in the country. MFT is responsible for running a family of nine hospitals across six separate sites, providing a wide range of services from comprehensive local general hospital care through to highly specialised regional and national services.

The post holder will be responsible for developing and leading the Pharmacy team to deliver high quality, patient focused clinical pharmacy service to the Medicine division of WTWA. This stimulating role will include rotations through the Urgent Care Directorate at Wythenshawe Hospital (Acute Medical Unit, Ambulatory Care Unit, Clinical Decisions Unit) and Acute Medical Unit / medical wards at Trafford General Hospital.

In this exciting, fast paced specialty you will deliver complex pharmaceutical care for patients, optimising their medicines and ensuring the effective transfer of care of medicines at discharge or onwards into other care settings. You will provide timely medicines reconciliation; prevent missed doses and support patient flow through the departments. You will be expected to participate in consultant-led post-take ward rounds, multidisciplinary medication incident review group, clinical guideline development while ensuring key governance priorities are met around high-risk drugs, polypharmacy, frailty and falls risk reduction in a growing local elderly population.

This post supports the new AMU medical model way of working at Wythenshawe Hospital and a clinical pharmacy service is provided between 7am – 6pm on this site. The post holder, as part of a team, will be expected to work flexibly over 4 days to support these hours on this site. You will also contribute to the out of hour’s (on-call) service and participate in the department’s weekend working and late night rotas.

There is a strong commitment to education, training and research in the department and close links are held with the Pharmacy Practice Unit within the School of Pharmacy at the University of Manchester.
