Job Description

Lincolnshire County Council are looking to recruit a number of enthusiastic and motivated Community Care Officers to support the Local Authority's commitment to provide effective and efficient services to improve outcomes for our service users. 

We need you to have a 'can do' approach to your work with good communication, Literacy and IT skills. Fundamental to the role is being able to work in partnership with our service users and their families, our health colleagues from both the voluntary and private sectors.

Are you able to undertake strengths based assessments in line with the Care Act 2014, complete Continuing Health Care Checklists and arrange cost effective packages of care for people who are eligible for funded services?

If so, we want you as a Community Care Officer at Lincolnshire County Council.

In this role you will hold and manage a caseload of service users and develop care packages to include the design of individually tailored care for service users in consultation with identified carers/ families. Enabling individuals and their carers to build on their strengths overcome obstacles and make informed choices. This in turn may lead to providing a range of equipment, arranging adaptation work and/or teaching alternative techniques to overcome the effects of disability on independent living where appropriate.

Depending on the post you may be required to travel effectively and within reasonable timescales. You will need to be committed to positive multi-agency work and have a good understanding of the challenges that may currently affect your chosen team.
