If you answer yes then we have the job for you. Willesden Centre for Health & Care runs 35 beds for general rehab and 20 beds of specialist neuro rehab. We are looking for a registered professional interested in discharge planning and who enjoys conversing with patients and their families to establish pre hospital needs v discharge needs. You will also gain skills in communication and liaison with our partner agencies in Local Authority and CCG. You will be joining a friendly team who want to help you develop you role whilst supporting the team to discharge patients and support flow through the organisation

Tier 2 Certificate of Sponsorship
